Telephone assessment, guidance and coordination during defined service hours, with clear response expectations.
Accompaniment to up to four prearranged on-island medical appointments each month, up to two hours each, in addition to regular home visits.

Medication reconciliation, provider and pharmacy communication, appointment coordination, and follow-up tracking.
Up to two additional 60-minute home visits each month when extra support is needed, arranged together and in addition to regular twice-weekly visits. Weekly updates keep family you authorize informed.
Our Health Care Assistant can help with personal routines, bathing and dressing, light meal preparation, and getting ready for appointments. This practical support can make everyday life at home more comfortable while helping you maintain your independence.
Visits are coordinated with your nurse and care plan, and billed separately at $75 per hour with a two-hour minimum.
Ask about HCA supportServices are subject to an initial fit assessment and written agreement. Longer appointments, care beyond the included visits and off-island appointment support are discussed and arranged separately. Unused visits and appointment support do not roll over.

This service may suit you if you are managing several health conditions, navigating a return home after hospital care, or looking for more continuity and help coordinating healthcare.
The membership focuses on nursing oversight and health management. Optional HCA visits add practical support at home. If your main need is daily personal care or continuous supervision, a service designed for that level of support may be a better fit.
No. You may be considered for membership without a family physician or nurse practitioner. I can help you understand and navigate available healthcare services. Membership does not include finding you a primary care provider, and nursing care does not replace a prescriber.
Monday to Friday, 8:30 a.m. to 4:30 p.m., excluding statutory holidays. Calls and secure messages are acknowledged within one business hour, with a clinical response within two business hours. After-hours messages are treated as received on the next business day.
Yes, with your consent. Together we identify who can receive updates and what information you want shared. Your preferences and autonomy guide the relationship.
Prearranged off-island clinical appointment advocacy is subject to availability, at $100 per hour with an $800 maximum for a service day of up to 12 hours.
No. There is no continuous monitoring, emergency dispatch or guaranteed after-hours care. Call 911 for an emergency. We establish clear escalation and continuity arrangements as part of your care plan.
